We do so by meeting and having in-depth discussions with all concerned persons. We listen attentively, solicit and encourage input from everyone. The information we obtain in these sessions will enable us to develop a comprehensive plan appropriate for the individual’s need for comfort, safety, medical care and independence. Moreover, we handle appointments and paperwork.
Spectrum Geriatric Care Management includes:
- Conducting care planning assessments to identify problems, eligibility for assistance, and need for services
- Functional assessments to determine if assistance is required for bathing, dressing, managing appoints, etc...
- Screening, arranging and monitoring in-home help or other services
- Assistance with memory loss issues
- Counseling and support for depression, anxiety, grief or loss
- Facilitation of family meetings- acting as a mediator between family members
- Discussing financial, legal, or medical issues and offering referrals to geriatric specialists to avoid problems and conserve assets
- Assisting with moving an older or disabled person to or from a care facility
- Accompany client’s to MD appointments and coordinate care between all service providers
- Acting as liaison to families at a distance, making sure things are going well and alerting families to problems
- Providing education and advocacy

Range of services
Some clients may need only an assessment to have the professional recommendations they are looking for. Some may need ongoing care management. Some may choose to use a care manager for a few hours a month, for concrete services or more complex issues. Others may want the care manager to manage all of the service needs of the individual so they have no worries. We cater services to each individual client.
When you are facing complex issues, a care manager is always recommended. Concerned family members are often heavily burdened by the demands of both work and family. It can be very costly to search for an appropriate facility for your loved one, especially for those who do not live nearby. It is much easier, advantageous and cost effective to use the knowledge, expertise and experience of a professional GCM who can provide you with an informed evaluation. The GCM is aware of the emotions involved in making important decisions about the care of your loved ones.
