What do Spectrum Geriatric Care Managers do for families?
What do you do when an aging or disabled family member has been discharged from the hospital and still needs significant assistance with care, has multiple medical problems, seems depressed or more? How do you figure out what is best for them? Where do you even start to access resources for help?
Help can begin with a phone call to a Spectrum GCM Care Manager. During this phone call, the presenting problem will be discussed so the care manager can understand the needs of the client and the family. The care manager will present the spectrum of services that can be provided and the fees and review the contract for services.
